ASTROCARE MENTAL HEALTH OUTPATIENT

PSCH Inc. a leader in the field of Behavioral Health and Human Services seeks experienced SOCIAL WORKER for Brooklyn Article 28 Clinic.

Certified by the New York State Department of Health, our Article 28 Clinic is a diagnostic, treatment and HIV Primary Care Provider that provides quality primary medical care to patients aged 6 weeks through adulthood.

ESSENTIAL ACCOUNTABILITIES:
Within this vital role selected candidates will complete comprehensive assessments of individuals to determine their needs and appropriateness for enrollment in Outpatient Treatment Program.

Provide written report findings.

Write and maintain current patient treatment plans.

Provide individual and/or group counseling services to patients and/or his/her collaterals.

Document treatment in session notes that document the service provision and include an assessment of progress and review of the treatment plan. Provide reports re: course of treatment, recommendations and outcomes.

Make written recommendations re: the patient need for related services.

Assist patients to access concrete services to the extent necessary.

QUALIFICATIONS
LCSW or LMSW, required.

Some experience working in an Article 31 or similar setting providing verbal therapy, required.

Must speak Creole and be culturally sensitive to Caribbean community.
